Allegations
While registered as a social worker:
1. You attended work while under the influence of alcohol and/ or Diazepam which affected your ability to carry out your role, on or around the following dates:
(a) 10 January 2017;
(b) 1 October 2020.
2. You have a health condition, as outlined at Schedule 1, which affects your practice.
3. On or around 13 June 2016 you were charged with the offence of ‘Fail to provide a specimen for analysis – vehicle driver 2016/06/13’ contrary to section 7(6) of the Road Traffic Act 1988 and Schedule 2 to the Road Traffic Offenders Act 1988’
4. On or around 13 June 2016 you failed to notify your regulator as soon as possible or at all, of the charge set out at paragraph 3
5. On or around 5 October 2017 you failed to notify your regulator as soon as possible or at all, that you were charged with the offence of ‘Fail to provide a specimen for analysis – vehicle driver 2017/10/05’ contrary to section 7(6) of the Road Traffic Act 1988 and Schedule 2 to the Road Traffic Offenders Act 1988
6. On or around 23 June 2022 you failed to notify your regulator as soon as possible or at all, that you were charged with the offence of ‘Fail to provide a specimen for analysis – vehicle driver 2022/06/23’ contrary to section 7(6) of the Road Traffic Act 1988 and Schedule 2 to the Road Traffic Offenders Act 1988
7. On 1 February 2018 you were convicted at Avon and Somerset Magistrates’ Court of failing to provide a specimen of breath for analysis pursuant to section 7 of the Road Traffic Act 1988, after you were suspected of driving a motor vehicle whilst over the prescribed alcohol limit.
8. On 17 August 2022 you were convicted at Swindon Magistrates’ Court of failing to provide a specimen of breath for analysis pursuant to section 7 of the Road Traffic Act 1988, after you were suspected of driving a motor vehicle whilst over the prescribed alcohol limit.
9. You failed to notify your regulator of:
(a) your conviction set out at paragraph 7 as soon as possible
(b) your conviction set out at paragraph 8 at all.
10. On or around 14 November 2022, you modified an email sent by Social Work England and/ or created an email to appear as though it was sent by Social Work England.
11. Your conduct at paragraphs 4, 5, 6, 9 and/or 10 was dishonest
The matters set out in paragraphs 1, 3, 4, 5, 6, 9, 10 and/or 11 amount to the statutory ground of misconduct.
The matter set out in paragraph 2 amounts to the statutory ground of adverse health.
The matters outlined in paragraphs 7 and 8 amount to the statutory ground of a conviction in the United Kingdom for a criminal offence
