A list of what you need to include in your concern.
What we need from you
We need as much detail from you as possible, for example:
- the name of the social worker, or other information we can use to identify them
- the social worker's registration number, if you know it
- details of the social worker's employer, such as name and contact details
- details of any complaints you’ve already made to the social worker's place of work
- a description of what happened, including dates and times
- an explanation for why you are raising this concern
- where things happened, including addresses
- details of other people who saw what happened, if you have them
If you don't know the social worker's registration number, you should be able to find it by searching for them on our online register.
If, after searching the register, you are uncertain of the social worker’s registration number, for example if there are multiple social workers with the same name, please write 'unknown' on the concern form.
We will use all the information and evidence that you share with us to decide whether to investigate, so it’s important to send us everything now.
