Whilst registered as a Social Worker, on or around February to September 2019:

1. You failed to demonstrate the necessary level of knowledge, skills or judgement of a social worker including in the following ways:

b) Providing inaccurate information to Service Users; *

c) Not gathering relevant information to inform assessments; *

d) Not recognising and responding to risk. *

The matters outlined in regulatory concern (1) amount to the statutory ground of misconduct and / or lack of competence or capability.

Your fitness to practise is impaired by reason of misconduct and / or lack of competence or capability.

The allegations are subject to an application by Social Work England to be discontinued in full. *
